Purpose of the Study: Severe bony and soft tissue defects are challenging problems for successful implants surgeries. The aim of this clinical case study is to use a custom Zirconia Mesh GBR to help reconstruct horizontal bony defect and use FGG graft to correct deficiency of keratinized gingiva tissues.
Methods: CBCT and digital scans were taken and a custom Zirconia mesh was designed and fabricated . Guided bone regeneration surgery were performed using allograft /PRP xenograft sticky bone and collagen membrane/PRF membranes following PASS principles. After guided implants placement, FGG soft tissue graft was completed following Zucchelli techniques.
Results: We were able to successfully completed bone and soft tissue augmentation. Two implants were successfully placed and restored patient's function
Conclusion: Although we had encountered complication of mesh exposure, custom Zirconia mesh was able to provide good space maintenance for GBR bone reconstructions and can be used for severe ridge deficient cases. The free gingival graft is an effective treatment in increasing the width of mandibular keratinized mucosa on the buccal side of the implant and in maintaining the health of peri-implant tissue.
