Purpose of the Study: ● One-piece dental implants (OPIs) are designed as a single unit with the abutment fused to the implant body. ● The prosthesis can be cemented to or press-fit onto the abutment. ● OPIs come in various diameters and lengths and are available with o-ball and crown-and-bridge prosthetic platforms. ● They are suitable for all-on-X cases and complete or partial edentulous situations, making them especially beneficial for anterior teeth. ● OPIs are ideal for stabilizing dentures and addressing limitations in interdental spaces. This presentation will: 1. Showcase different types of OPIs, 2. Discuss the advantages and disadvantages of OPIs, 3. Examine the factors that contribute to the success of OPIs and optimum placement location of OPIs 4. Present OPI cases with immediate loading and function.
Methods: CBCT scan, surgical instruments, Implant surgical kit, local anesthesia.
Results: Many publications have shown that OPI yields a high success rate and can provide immediate function (2,3,4,5,8,9). Minichetti et al (2) placed 33 titanium Zimmer OPIs in various tooth locations of 24 patients, using Zimmer OPI with 2.4 mm to 4.7 mm diameter. All were completed with ceramo-metal crowns after 3 months of provisional non-occlusive restorations, protecting OPI for osseointegration. Implant survival rates were 100% after 2.6-10 years of follow-up. The report described single unit cases but not multiple units OPI nor “all on X” cases. Hahn (4) evaluated the radiographic outcome of 47 Nobel Biocare titanium OPI with immediate function in 30 patients. Implant diameters range from 3.0 mm to 5.0 mm. Implant lengths were from 10 mm to 16 mm. 47 implants were placed in different tooth locations; however, only 2 implants were placed in the anterior mandible. Radiographs demonstrated good marginal bone levels and documented a survival rate of 97.9% after up to 4 years of loading. 18 of those 47 implants had been placed immediately after tooth extraction with flapless technique. Local bone grafting was performed. All implants were immediately provided with temporary restorations (4,5).
Conclusion: OPIs can be placed using full-thickness flaps or flapless techniques. With careful case selection, treatment with OPIs can deliver prostheses that provide for aesthetics, function, comfort, and long-lasting results.
